Note:
The health clearance permit is valid for 6 months from the date of issuance.
Process Steps
Step 1 |
The Applicant proceeds to complete the application form, he/she must also gather all supporting documentation. |
Step 2 |
The Applicant then pays the prescribed fee and submits application form with supporting documents to the policy registry at the Ministry of Health (MOH). |
Step 3 |
The policy registry then forwards the application to the Director responsible for food safety for review. |
Step 4 |
The Director responsible for food safety then forwards for review to the Assistant Director responsible for food safety who then reviews, signs and forwards the
application to the Chief Environmental Health Officer for food safety. |
Step 5 |
The Chief Environmental Health Officer for food safety then reviews the application and requests for additional information from the applicant if need be or issues |
